What to Do When Someone Is Choking: A Simple Guide for Babies, Kids, and Adults

By Dr. Roberto Iglesias, MD, FACS

Choking can happen in seconds at a dinner table, a birthday party, or in the back seat of the car. The good news is that the steps to help are simple, and anyone can learn them. You don't need medical training. You just need to know which technique fits the person in front of you.

First, is it really choking?

Someone who is coughing hard, talking, or crying is still moving air. Stay close, encourage them to keep coughing, and don't hit their back or start thrusts yet. A strong cough is the body's best tool.

Act right away if the person:

Have someone call 911 right away.

Babies under 1 year: back blows and chest thrusts

A baby's body is small and fragile, so never use belly thrusts (the Heimlich maneuver) on a baby under 1. Use this instead:

  1. Sit down and lay the baby face-down along your forearm, with your arm resting on your thigh. The baby's head should hang lower than the chest, so gravity helps.
  2. Support the head by holding the jaw, not the soft throat.
  3. Give 5 firm back blows between the shoulder blades with the heel of your other hand.
  4. Turn the baby face-up onto your other forearm, still keeping the head low.
  5. Give 5 chest thrusts: put two fingers in the center of the chest, just below the nipple line, and press down firmly about 1½ inches.
  6. Keep repeating 5 back blows and 5 chest thrusts until the object comes out or the baby starts crying, coughing, or breathing.
  7. If the baby goes limp or stops responding, start infant CPR and call 911 if no one has.

Children over 1 and adults: abdominal thrusts (the Heimlich maneuver)

  1. Stand behind the person. For a small child, kneel so you're at their height.
  2. Wrap both arms around their waist.
  3. Make a fist and place the thumb side just above the belly button, well below the breastbone.
  4. Grab your fist with your other hand.
  5. Give quick, hard thrusts in and up, as if you're trying to lift them.
  6. Keep going until the object comes out or the person can breathe, cough, or talk.
  7. If they pass out, gently lower them to the floor, call 911, and start CPR.

How to help a choking child over 1 or adult: abdominal thrusts

When to change the technique

Pregnant or larger people. If you can't reach around the belly, or the person is visibly pregnant, use chest thrusts instead. Wrap your arms under their armpits, place your fist on the center of the breastbone, and pull straight back hard. This protects the baby and works when your arms won't fit around the waist.

If you're alone and choking. Call 911 if you can, even if you can't speak. Then give yourself abdominal thrusts with your own fist, or lean over the back of a sturdy chair or a countertop and push your upper belly hard against it.

A few important don'ts

After the scare: get checked

Even if the object comes out and everyone seems fine, see a doctor afterward, especially after abdominal or chest thrusts. Those thrusts can bruise or injure the belly or ribs, and a small piece of food can sometimes slip into the lungs. A persistent cough, wheezing, fever, or trouble breathing in the days that follow needs medical attention.

Prevention goes a long way

My best advice: take a class

Reading this is a great start, but practicing on a mannequin builds the muscle memory you'll need when your heart is racing. The American Red Cross and the American Heart Association offer CPR and first aid classes in a couple of hours, often on weekends. It's one of the best investments you can make for your family.

No necesita entrenamiento médico.
